Loan Application Checklist
  • W2 tax forms – most recent two years
  • 1040 tax forms – most recent two years (all schedules, please sign the second page of the tax return if it is a copy)
  • Current pay stubs from the last 30 days
  • 2 months of bank statements (all pages)
  • Most recent copy of 401K or IRA statement
  • Copy of the HUD settlement statement and the Note from the most recent refinance/purchase
  • Current mortgage statement
  • Copy of the homeowners insurance declaration page or the agent’s name and phone number
  • Copy of your driver’s license and Social Security card (The copy needs to be clear enough to read the numbers on the page)
  • If you’re a business owner also include:
    • K1 tax form – most recent 2 years
    • Business tax returns – most recent 2 years
